Start from a ready-made form.
They are short, tested and already connected.
Ask one thing per question.
"What's your email?" works better than "Name, email and phone?".
Keep it short.
Every extra question loses some people. Make only what you really need required.
Greet people by name.
Insert their earlier answer: "Thanks, {name} — where can we reach you?".
Use logic to skip what does not apply
instead of asking people to skip it themselves.

If an answer should become a lead or a ticket, let the form create it — nothing gets forgotten in an inbox.
Update leads instead of duplicating them
